| Your Appointment |

You will receive the best results from your therapy when you attend as scheduled by your therapist. We know life can be hectic and unpredictable. If you cannot make your appointment, please provide 24 hours notice so that we can schedule someone else. We charge $50 for late cancellations or no-shows. |

Please arrive on time for your scheduled appointment. If you are early, lets us know and depending on availability, we may be able to start your session early. If you arrive late your treatment time will be shortened accordingly or you may have to reschedule. |
| Your Attire |

You should dress in comfortable loose fitting clothing similar to what your wear to exercise. If you are receiving treatment on your lower body, shorts should be worn. A loose-fitting, short-sleeved tee shirt would be suitable if you are receiving treatment for an injury to your upper body. Please do not wear jeans, dress shoes, or formal wear. We do have changing rooms available for your use. |
| Working with Your Physician |

We like to work closely with your physician. Let us know if you are going to visit your doctor and we can fax a report of your progress. If you should need a new prescription for your therapy, as required by some insurance companies, it can be faxed to us by your doctor. Our fax number is (484) 582-0666. |
| Your Treatment |

The first thing we do is get a baseline of your health by reviewing your medical history, including surgeries you have had and medications you are currently taking. We also do a comprehensive evaluation where we understand your injury and what we may be able to do to relieve your symptoms. Communication is key to maintaining your progress and identify changes to your symptoms. We must be kept informed of anything that may impact your treatment. |

We may ask you to do some exercises at home. This will help speed your recovery. There are additional things we may ask you to do including stopping any activities that may affect your injury. It is also important that your keep your therapist informed of any changes in your regular activities |
| Payment |
| |
We will be glad to look up your benefits, but ultimately it is your responsibility to know your coverage and co-pay. Co-pays are expected at the time of service. |

If you are a Medicare patient it is your responsibility to know how much has been billed for therapy in the current calendar year. |

Payment options are available for those qualified. |
